Welcome to Riverbend Therapeutic Alliance.
Riverbend Therapeutic Alliance is currently accepting patients between the ages of 21-65 with Aetna, Cigna, ConnectiCare, Optum, Oxford, and United Healthcare for medication management.
Please complete the new patient screening and intake forms below. One of our providers will review it to be sure we are able to meet your therapeutic needs.
If you plan to pay out-of-pocket, please compete the following form. If you have in-network insurance (Aetna, Cigna, ConnectiCare, Oxford, or United Healthcare), you do not have to fill out this form.
You will receive an email from admin@riverbendtherapeuticalliance.com to set up your patient portal or an automated text.
Download MyIO app from store or Google Play.
Please add to portal immediately your HSA or Credit Card. You will use the app MyIO frequently to communicate with your provider, view your billing statements and for the Telehealth for visits. We highly recommend enabling notifications so you do not miss the beginning of an appointment or a response from your provide around a clinical concern.
To assist in verification of your insurance benefits (if you are not a private pay patient), you can send a photo with your phone of front and back of your card to admin@riverbendtherapeuticalliance.com.

- Download MYIO app to your phone or use via desk top.
- Complete portal set up, making sure to add credit card (HSA is preferred) to your account under billing.
- If using insurance benefits, send in a copy of your cards to our administrator for benefit verification.
The more expedient these items are completed, the sooner that you can be scheduled for an appointment. Please note, after our providers review your jot form—if we are not a good clinical fit—you will get an email with realistic in network options that can assist you with an appointment in less than 2 weeks.
